Replaced: Unlock the Reflashed Achievement Guide
To unlock the “Reflashed” achievement you will need to “complete three hacking sequences without mistakes.
You won’t be able to do this achievement until Chapter 6 when the hacking ability unlocks after a guard gets zapped by an electric fence. Pretty much as soon as you get it, you should be able to unlock this achievement though.

Hacking is indicated by a little green flash going off on Reach’s coat. This indicates that something nearby is hackable. Press and hold LT (Xbox controller) to take out the hacking device and press Y to hack. This will trigger a mini-game on your Wingman.
The aim is simple, you must complete the box by finding the tile from a selection on the right. The tile must line up and fill in the blanks of the tile on the left.
You only have a short amount of time to complete each puzzle, indicated by the bar at the bottom of the screen.

There are three of these to complete for each one hack. You need to complete a full hack three times for the achievement. The first one you do as part of the tutorial counts. As you progress, you’ll need to hack turrets to get past. Doing these hacks without mistake should net you the achievement.
Here’s a video showing the hacking and unlocking the achievement:
