The third instalment in the Pathologic series is getting a free prologue, called Pathologic 3: Quarantine, and it’s releasing on March 17th. The announcement was made by the game’s developer Ice Pick Lodge and publisher HypeTrain Digital.
Pathologic 3: Quarantine is described as a “narrative-driven adventure about a young scientist in search of immortality and a mysterious plague outbreak in a small town” on the prologue’s Steam Page. You play as Bachelor Daniil Dankovsky, a young scientist from the Capitol that seeks immortality. After hearing rumours of an immortal man, he travels to the Steppe to see for himself. On arriving, he is confronted by a mysterious sickness affecting the locals, known as the Sand Plague. “Examine patients, uncover strange secrets, and make first steps towards immortality.”

The third instalment in the Pathologic series was announced last year, and deals with the same premise as its predecessors, with some changes. Pathologic 3 and its prologue have you playing as a single character, while the previous two had multiple protagonists that provided different perspectives and gameplay.
It seems that the newly announced Pathologic 3: Quarantine will work as a way to introduce players to its new gameplay, focused around “examining patients and diagnosing them, but also investigating their life situations to find out the unpleasant truths they may hide.” Check out the trailer below to get a sense of what is in store.
It will be interesting to see how much of the series’ complex meta-narrative and punishing gameplay will be present in Pathologic 3: Quarantine. While the games do vary from one another, the team at Ice Pick Lodge clearly feel there is more to explore in their critically acclaimed cult IP.
Pathologic 3 is due out for PlayStation 5, Xbox Series, and PC via Steam in 2025.
