The Tease of the Decade: ILL Exists?

There is always those titles in the gaming space that look too good to be true. Ones whose initial trailers invoke more questions then answers about what they’ll actually be about. ILL was one of those games.

Originally announced back in May of 2025, many of us in the horror community were infatuated by the gore and disgusting looking enemies that were shown to us on our screens all those months ago. With that same level of curiosity came a certain sense of dread. The trailer looked like a proof of concept more than an actual game, and in the gaming space this is often a sign of a game destined to stay in development hell. It appears we have escaped that fate for now. Thanks to the State of Play we now have some real gameplay and some hints about the tone the story is going to follow.

The Premise: A Coma Patient From The Moon?

The trailer is pretty broad in the information it gives us but what it does do is give us a glance into the beginning of the game. The character we play as is a man who wakes up from a coma who looks as if he has been attacked by some sort of beast. The scene then switches to what looks to be a morgue of some kind complete with the same disgusting bloody zombie like enemies we saw from the first trailer.

If anything has been consistent in this game so far, it’s the sheer amount of blood and guts that seems to exist on the screen at all times. Throughout the rest of the trailer, we are quickly introduced to a few other characters all of which seem dumbfounded that our character is here at all. The most puzzling words said by one of them, “You fell from the moon”, seems to hint that there is something more going on here besides the slaughter of the usual fiendish undead. Either way my curiosity is peaked to the highest degree and I can’t wait to see what information we get next.

The darkened halls of some kind of industrial plant are the perfect setting for ILL.

ILL is a realistic first-person action horror game that takes you into a dark fort overtaken by a mysterious entity. Fight against unpredictable monsters, a visceral dismemberment system, and realistic physics create an atmosphere of relentless terror. ILL is slated to release in 2027 on PC, PS5, and Xbox Series X/S.
