The collectible grind rolls right along! We've been going through collecting them all, so here are all the readables in Chapter 5 – Everything That Rises. There are 11 in total, as well as 1 health filament.
1/11 – Burrower's – Pieces of the Self
As soon as you find Crouton and learn to take control of him, before you enter the hollow log, look right beside it. There will be a Readable on the tree beside it.

2/11 – Statue Flyer
Continue until you reach a giant statue and a big sign that says "Auntie Belle's". Just before the sign, there is a piece of fence still standing. On the fence is a Readable.

3/11 – Burrower's – Iron
After meeting at the church, look right towards the path you need to take. At the broken fence will be a wooden crate with a note on top.

4/11 – Hollow Dreamer
After the previous collectible move forward and after crossing a bit of water, you will see a house ahead of you to the right. On the couch on the front porch is the next Readable.

5/11 – Kids Need A Hand
After clearing the second big piece of Stigma and watching the scene, the home will open up. Inside, on the table, is a Readable.

6/11 – Jo's Penance
From the previous collectible, head upstairs into the bedroom. On the dresser will be another Readable.

Health Filament
This isn't a readable collectible, but back outside, look to the left as you leave. You will see a grapple point you can take across to a small piece of land. Use the ghost objects to jump up and grab a health filament if you want.

7/11 – Yard Sale Notice
Quite a bit past the last collectible after avoiding some....well shit sprayers, you will reach an old farm (it's part of the main quest). Once here, you will see a large windmill which you can climb. There will be a ghost box you can use to get on top.
Reach the top, then jump over to the stack of hay and over the fence. Follow the path, then turn right to see a cabin. The Readable will be on the porch on a chair.

8/11 – Honor thy Father and Mother
From the previous collectible, head back toward the main path up to where the creepy-looking house is. The front door will open when you approach. Once inside, hang a right into the living room. The note will be on the coffee table in front of the couch.

9/11 – A Bashful Farmhand in the Setting Sun
From the previous collectible, head up the stairs. Head around the hall to the room on the right. On the desk, you will find the next Readable.

10/11 – The Pigs are a Loss
After filling the bottle and making it to the tree, you will come to a section where you need to use Crouton to get inside the pig factory and power the door. After opening the door and going through, as soon as you get into the bigger section, look to your immediate right. On a table will be your next Readable.

11/11 – Sutter's Will and Testament
After defeating Two-Toed Tom and going inside of him, you will have to jump on a raft being towed around by a pig. Once you get to where you can jump up, do so. Go right and find the Readable on the ground as soon as you do.
