Alright we're here at chapter 12 and this one is the biggest of all the chapters as far as collectibles are concerned. The chapter features a whopping 19 collectibles as well as the final health filament. Strap in, you are going to be here awhile.
1/19 – Should I Leave?
Right from the beginning of the chapter we're grabbing collectibles. Head forward until you get to the crossroads. On a tree stump to the right of the sign post will be the first note.

2/19 – Should I Stay?
Look at the sign and follow the correct direction into the fog. You will know you've gone the right way if Hazel says "this is new". You will be back where you started but things will have changed a bit. In the exact same spot of the first note will be another note. It has the same title but it is indeed new.
3/19 – The Record Keeps Skipping
Enter the wagon and walk forward. You will see an old record player with a note attached.

4/19 – Milk and Sugar, Love?
These are coming quick! Walk forward slightly to find another note on a table with a tea pot and some cups.

5/19 – Ring, Ring
Head slightly forward once more to find a tricycle with a note attached.

6/19 – Roux's Limerance
Head up the stairs now and directly infront of you on the table will be yet another note.

7/19 – The Hungry Maw
Turn right and infront you will be what I think is supposed to be a fridge? On it will be another readable.

8/19 – Still Stars Don't Remember
After the first battle and clearing the stigma look to your right. You will see a little gated area you can jump over to and there will be a crate you can move around to get to a floof. To the left of the crate will be a note on a table.

9/19 – Joe Goodfellow, 74
Jump across the gap and before taking the wind tunnel look to your right. You will see a door with what looks like a poster on it which will be the next readable.

10/19 – The Many Little Things
Take the wind tunnel and as soon as you land look to your left. On a chair will be yet another note.

11/19 – Hollowed Love
Continue forward for about 2 seconds. On a table on the left of this little corridor will be another note.

12/19 – Spit It Out
Wall run and take the next wind tunnel up. Instead of going forward turn right. At your first chance turn right again to go behind the building. Behind it on a table will be the next note.

Health Filament
Continue past the previous note and make another right again. You will see ghost platforms that you can activate and jump to. Follow them along to the wind tunnel and grapple point all the way to the top. You will eventually get to a large platform with the final health filament on it. This should unlock the achievement for max health if you've been following along.

13/19 – Ella May, 7
Head back to the main path now and jump down. Follow along and jump across to another section with one of those doors similar to earlier. There will be a note on this door.

14/19 – I miss, I miss, I miss
Turn around 180 degrees from the previous collectible and look down. You will see a ghost platform you need to activate. Do so and jump down. Once on it quickly look right and up into the little alley for a grapple. Grapple up to find a floof and in behind the floof is another note on the table.

15/19
This might be one of the trickier ones in the entire game. Complete the arena battle and continue on up the path. As soon as you go up a very small flight of stairs turn directly around. You will see a roux sign above that you can pull down (I've already pulled it down in the pic below).

Jump to where the sign was and make a left on the walkway out to an area with a floof. To the left of the floof is a note on a chair.

16/19 – Tempest Hamlin, 42
Progress now up the next set of stairs. At the top of the stairs on the left hand side will be another one of those windows with a note.

17/19 – Passage
Progress until you get to the clock tower. Head inside the clock tower and activate the gears and go up the first one. Once you drop off the gear follow the path. You are going to need to throw a gear to progress but beside the box where you can get the gear is a note on a workbench.

18/19 – I'm trying
Continue up the clock tower jumping through some moving gears and grappling up. Once you grapple to what looks like an elevator shaft continue along the path and you will find a note on a stool infront of a gear leaning against the wall. This one is pretty hard to miss.

19/19 – You Gotta Release It
Once you exit the clock tower and get back outside make a left to start going around the back of the tower. On a crate will be your final readable of the chapter.

That's the end of the chapter in terms of collectibles. Sheesh that was a lot wasn't it? If you've been following along you should have 101/103 now. Can you see the finish line? I sure can.
